Dog License Information:

All dogs in the Town of Sevastopol are required to be licensed. To obtain a dog license, please send proof of vaccination and payment to:
TOWN OF SEVASTOPOL
P.O. BOX 135
STURGEON BAY, WI 54235-0135
$3 Neutered male / Spayed female
$8 Intact male / Intact female
$35 Kennel (first 12 dogs)
PROOF OF VACCINATION must include name, sex, color, breed, date, expiration and manufacturer of vaccine (it will be returned to you with your dog license).

Driveway Permit (no fee): You can download the following form, complete and return to the Town office.  Please stake or mark your driveway so that the location can be identified and determined if a culvert will be required.  Also see "Ordinances" link for Town road and driveway standards.
